I am a newly qualified nurse working in ITU. When it comes to documentation I often get in a rut when trying to write down my patient assessment. Sometimes I forget to use the A-E approach and thus leave important information out. I have created these sticky notes for convenience. Simply document your patients information using the structure provided, peel off and pop into the patinets notes. This provides an easy way to document as you work on the shift, without having to start writing in notes, when you are not ready to do so.
